Absentee Ballot for Village Elections

Copies of birth certificates may be requested using the Birth Certificate request form in our Forms Center. Please mail your form to the Village Clerk’s Office, 120 Larchmont Avenue, Larchmont, New York 10538. Enclose a copy of your identification, along with a check in the amount of $10 per copy requested. No information may be given over the phone or by e-mail.
Who is eligible to obtain a birth certificate copy?
- The person named on the birth certificate.
- A parent of the person named on the birth certificate (requesting parent’s name must be on birth certificate).
- Only by order of a New York State Court may a spouse, child or other persons obtain a copy of a birth certificate.
Copies of death certificates may be requested using the Death Certificate request form in our Forms Center. Please mail your form to the Village Clerk’s Office, 120 Larchmont Avenue, Larchmont, New York 10538. Enclose a copy of your identification, along with a check in the amount of $10 per copy requested. No information may be given over the phone or by e-mail.
Who is eligible to obtain a death certificate copy?
- The spouse, parent or child of the deceased
- Other persons who have a:
- documented lawful right or claim
- documented medical need
- New York State Court Order
What is a lawful right or claim?
- If the applicant is not the spouse, parent or child of the decedent, a lawful right or claim must be documented. An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.

FOIL request applications may be found in the Forms Center portion of our website. They should be submitted to the Village Clerk’s Office once they are completed.
The Village of Larchmont neither issues nor maintains marriage license records. Only Cities and Towns in New York State have this authority. The nearest communities to Larchmont include the City of New Rochelle and the Town of Mamaroneck.
Land Use Board Appearance
Contact our Building Department when you’re considering a project that may require an appearance before the Planning Board and/or Zoning Board of Appeals. They can help you with the process and tell you what is required to apply.
If you’re interested in appearing before the Board of Architectural Review, the instructions and application form may be found in our Forms Center. Submit the application and the required documents to the Building Department. For submittal deadlines and meeting dates, contact the Building Department directly.
Permits – Most Types
Applying for most types of permits in the Village of Larchmont is easy! Visit our Forms Center, download an application, and bring it in to the respective department with the appropriate documentation and payment. For special events, application must be made at least three months in advance of event.
Tax grievance forms are available in our Forms Center. To grieve your Village of Larchmont taxes, an original and five (5) copies of the application, and any substantiating documentation, must be submitted to the Village Clerk’s Office in February of each year, but prior to the third Tuesday in February. On that evening, the Board of Assessment Review meets to review the tax grievance submissions and make their determinations.
Voter Registration Forms
To obtain a voter registration form in either English or Spanish, visit the Forms Center. Registration forms must be sent to the Westchester County Board of Elections in White Plains, New York.
